Orphan: First Kill is a 2022 American psychological horror film directed by William Brent Bell, and is a prequel to the 2009 film Orphan, about a couple who decide to adopt a female child after their son's death but discover suspicious things about her, where she initially appears gentle and tends to isolate, while she begins to admire the Lord of the house. At the end of the film, it turns out that she suffers from a disease that causes her body to not grow, and her real age is 34 years.

The second part comes under the title Orphan: First Kill as an introduction to these events, which shows how Esther managed an escape from a psychiatric facility in Estonia, and then travels to America by impersonating the missing daughter of a wealthy family to confront a mother who is ready to protect her family at all costs.

The project was announced in February 2020 in which Foreman reprised her role as Esther, and co-starred alongside Julia Stiles, Rossif Sutherland, and Matthew Finlan. Filmed in Toronto from November to December 2020. Orphan: First Kill will premiere in the United States on August 19, 2022.
